I am trying to design a custom board based on the BGM121 antenna module. After getting the design verified and assembling it per the guidelines, I am not able to get a Bluetooth signal. I am able to connect to and program the chip with soc empty but am noticing I get a flat line at around 2 mA after doing so (there are no peaks to indicate any sort of radio activity). What are some possible reasons for this?

Hi,
Did you also flash the bootloader? That is a common problem when trying to bring up custom hardware.

Yes I flashed the bootloader by using soc empty from the launcher screen and then uploaded the program itself. Are there any other explanations for this?

If you are able to debug the board, use the Pause buttton in the debugger and see where it is at.

I guess you just created soc-empty for the OPN you are using, compiled and flashed it. The sample apps don't have the bootloader, you have to create the bootloader project separately and flash it to the board. Please refer to UG266: Silicon Labs Gecko Bootloader User’s Guide.
